A woman doesnt have "milk" in her breasts while she is pregnant. There are "milk ducts" that produce milk in the breast and while pregnant these ducts become enlarged and ready to produce milk. usually after 28 wks your breasts begin to leek an off white coloured fluid called colostrum (colostrum is the start of milk). But it doesnt develop fully until the baby sucks on the breast for the first time. Its full of vitamins and minerals and has a thick consistency.
As baby takes more of the colostrum from the breast, the milk thins out and full production of the milk begins.
A woman can breastfeed for as long as she wants. Anywhere over a year is quite common. But it usually is not recommended after 18 months as babies/toddlers of that age should be getting used to full solids.
If a woman doesnt use her milk production she can get a breast infection (called mastitis) and her milk can dry up.
